Hello!

I couldn't see that the last option is missing a comma, sometimes I find it difficult to answer that kind of questions. Should I expect this kind of question in the real exam?

Generally, the questions won't test you on typing mistakes such as unmatched brackets, or missing semicolons etc. However, if something is a key part of the syntax then you may be tested on that. In this question, comma is a key part of separating arrays for inner dimensions while initializing multiple arrays. This is not a typing mistake and you should have caught it.
Similarly, if you are writing a lambda expression with 2 parameters, then you have to put them in brackets i.e. (a , b)->a+b; Here, brackets are important part of the syntax.

The question mentions initialization.
The third answer does not initialize the array fully, but it is still valid.

My question is: if this would only instantiate and not initialize any part of the array (but still it's fully automatically initialized with zeroes), could we say that the answer contain initialization?

It is initialized fully...just not explicitly. So, yes, it is still initialization.
In the exam, you may have to interpret what the problem statement means based in the context of the given options. If you face an ambiguous sentence in the exam, observe the options and the number of correct options that you have to select and then decide what the question is trying to get at.
